OCES aims to support elders and individuals with disabilities by providing vital information and coordination of services that promote healthy, safe lives for our consumers. We want to help them stay as active and engaged in their lives and in their own communities for as long as possible. To that end, OCES is looking for a dedicated, talented Registered Nurse to facilitate successful discharge activities and other transition support activities as a member of the Transition Support Team serving elders and people with disabilities. Perform nursing facility clinical determination activities completely and timely. Weekdays with flex schedule options, hybrid environment! How you'll make an impact: Assisting with discharge planning and transition support activities Conducting visits to all Skilled Nursing facilities in OCES' area to meet with discharge planners and consumers Determining consumers' MassHealth clinical eligibility/ineligibility for nursing facility services; determine duration of short-term approvals Determining applicants' eligibility/ineligibility for other transition support programs and activities Maintaining knowledge of current OCES policies and standards, and funding source regulations What you bring to the table: At least two years' health related clinical, supervisory and/or administrative experience Current MA RN License Degree in Nursing can be substituted for experience; A passion for working with older adults and individuals with disabilities Willingness to learn and grow with OCES.
